1. Personal information or third-party provision record requested shall be those which A&T has authorization to disclose.

2. Only requests for disclosure made by the customer or his/her agent will be accepted. Requests must be made via email (see CONTACT for email address), post or by visiting A&T.

If sending request by post, postage shall be covered by the customer.

3. Required documents and fees:

a) Application form

• Personal Information Disclosure Request Form (Inquire CONTACT)
• To request usage of personal information: Fill out and submit Request for Usage of Personal Information Form to A&T.
• To request disclosure: Fill out and submit Request for Disclosure Form to A&T.
• To correct, add, or delete information: Fill out and submit Request for Change in Personal Information Form to A&T.

b) Forms of identification

Driver’s license, health insurance card, passport.

c) Request made via an agent

Legal representative: document verifying the identification of the legal representative.
Power of attorney agent: power of attorney, certificate of seal impression.

d) Fees

To request for usage or disclosure of personal information, processing fee (actual cost) in addition to postage will be charged. No processing fee is required for requests made to correct, add or delete personal information.
(When requesting notification or disclosure of usage purposes simultaneously, processing and postage fee for only one request will be required.)

Payment method:

1. Registered mail
2. Stamps or postal money order without receiver indicated
3. Cash paid at A&T (Customer Complaints Regarding Personal Information)
4. Money transfer to our designated account

In case payment amount is insufficient, you will be contacted and requested to remit the remaining amount. However, if full payment is not received within one week, your request will become invalid.
Processing fee will not be refunded if A&T does not possess your personal information, or any of the reasons for nondisclosure applies (see 6. below).

4. Method for responding to disclosure requests:

A&T shall respond by delivering a written document or a method the principal demands pursuant to the provisions of laws etc. (in cases where disclosure by the method requires a large amount of expenses or when disclosure by the said method is difficult, by a method of delivering a written document). This also applies when request is made by an agent.
When the obtained personal data does not exist, or, when disclosure by a method a principal demanded pursuant to the provisions of laws etc. is difficult, A&T shall inform a principal thereof without delay.
When request is made by a legal representative, A&T will respond to the legal representative.

5. Use of personal data obtained relating to requests of disclosure:

Personal data obtained will be used only for the purpose of request of disclosure and will be destroyed after 5 years.

6. Reasons for nondisclosure of personal data:

a) Unable to confirm principal’s and/or agent’s identity due to incomplete documents.
b) Unpaid fee resulting in invalidated request.
c) When there is a risk of harm to life, body or property of the person or a third party.
d) Personal data acquired by subcontracting, and special care-required personal information will not by disclosed.
e) When there is a risk of considerable obstruction to A&T business operations.
